Executive Summary
Rakan KKM is a whole-of-government partnership to enhance Ministry of Health hospitals and clinics for all Malaysians. Inspired by the spirit of Malaysia MADANI, the objective of Rakan KKM is to create a win-win-win situation in the public healthcare system where we:
- increase the retention of healthcare workers to benefit all patients
- raise the floor, especially for the B40, by increasing investment in the public healthcare system
- raise the ceiling, especially for the M40, by providing more value-based options amidst rising healthcare costs
Rakan KKM offers 'premium economy' services at selected public hospitals for elective outpatient, daycare, and inpatient services, including:
- personalised care
- specialist choice
- enhanced privacy
In contrast to previous initiatives, Rakan KKM includes the entire healthcare team in its income support model, not just lead specialists. Crucially, excess revenue will be reinvested to improve public healthcare services for all patients.
This ensures that Rakan KKM is not a zero-sum game but rather contributes to the overall strength and sustainability of the public healthcare system.
Rakan KKM will start with a few hospitals and gradually expand to most major specialist hospitals across the country.

About Rakan KKM
Rakan KKM is a whole-of-government initiative to enhance MOH hospitals and clinics for all Malaysians. This MOH partnership brings together patients, healthcare teams, and government-linked investment companies (GLICs), facilitated by the Ministry of Finance through the GEAR-uP initiative.
At the heart of Rakan KKM lies our unwavering commitment to preserving and enhancing meaningful and affordable access to public healthcare for all–where only minimum user fees are required to access MOH services. We recognize the challenges our healthcare system faces, particularly in retaining our talented healthcare professionals in the face of competition from the private sector. Rakan KKM addresses these challenges head-on while ensuring that the core principle of accessible public healthcare remains intact.
As a partnership, Rakan KKM benefits all partners. Patients who choose to partner with us through Rakan KKM will have access to the same high standard of clinical care but with ‘premium economy’ services–personalised care, the ability to choose their doctor, and additional privacy and comfort, all at affordable rates.
These services will be available for outpatient specialist consultations, elective day procedures, surgeries, and inpatient admissions. Importantly, by opting for these services, our patient partners can be assured that they will be directly contributing to the strength and sustainability of our public healthcare system, and not benefiting at the expense of other patients–as they would be cross-subsidising and supporting services for all patients, not least through the retention of health workers.
All patients, including those who opt not to participate in Rakan KKM, will benefit through reinvestments to improve equipment, facilities and services, especially as limited government financing can now be focused on them, given that Rakan KKM patients will pay their own way. This means faster and improved services for everyone, as we optimise our resources and expand our capacity, including out-of-hours capacity.
This is not a zero-sum game as Rakan KKM patients do not benefit at the expense of other patients by cutting their queue. By optimising services and partnering with GLICs, Rakan KKM will drive investments to boost the capacity of MOH hospitals to serve all. Furthermore, Rakan KKM only applies to elective, not emergency services. Prioritisation and access to emergency services will be based on clinical needs and not the ability to pay.
For our health workers, Rakan KKM is our commitment to allowing our dedicated health workers to retain their public orientation while practicing their profession within MOH hospitals to the highest degree of excellence, through exposure to diverse patients and an interesting mix of cases, while working with the dignity of increased compensation, better-equipped hospitals, and high performing healthcare teams.
For GLICs partnering with Rakan KKM, our value proposition is exposure to a dynamic and growing sector, access to the unique proposition of a vast geographic network of public hospitals operating as clusters, and the top clinical leaders nationally. This will enable competitive returns on investment with the added assurance that beyond mere financial profits, good corporate citizenry has been exercised.
The best elements of the existing Full Paying Patients (FFP) initiative will be used eclectically to ensure that the public interest is served. However, there will be substantial changes from the old FPP model particularly to ensure that the whole healthcare team, not just one specialist, benefits. This is critical to strengthen our values and foster teamwork. The best healthcare is delivered by strong multidisciplinary teams and this is a key element of the ‘Rakan’ concept. A further change from FPP is the emphasis on investing back in improving the MOH hospital so that better services can be delivered for all.
Rakan KKM is not a step towards privatisation–rather, it is a strategic approach to preserve the public orientation and ownership of our MOH hospitals, in the face of increasing demand and financial constraints. Excess revenues will be reinvested in our public healthcare services. Serving the public interest also implies adherence to clinical best practices and the rational use of healthcare, rather than short-term profits. By doing so, Rakan KKM serves as a transformational agent to catalyse value-based healthcare throughout the ecosystem.
We plan to implement Rakan KKM as soon as possible, starting with a few hospitals in selected urban centres, and expanding to include most specialist hospitals across the country.
As Rakan KKM matures and expands, we envision that the morale and performance of our healthcare teams will reach new heights, bringing the best healthcare to all our patients. We envision expanded access to affordable ‘premium economy’ options. And most of all, we envision reinvigorated and modernised MOH hospitals and clinics which will continue to provide high-quality access to public healthcare for all in the future.

Services under Rakan KKM will not compete with the luxury and price points of existing private healthcare services.
